If you are being treated in our office due to an auto accident, please print and fill out the PI Intake Forms linked below. Bring them with you to your first appointment, with any other applicable documents (X-Ray, MRI, Accident Report, etc.).

Your first appointment could take up to 1 hour. To avoid delay, please have all applicable paperwork (available from the Personal Injury Intake Forms button above) filled out at the time of your appointment.

Recovery doesn’t end when you walk out our doors. To get the most out of your treatments in our clinic, we would like to provide you with rehabilitative exercises and stretches to complete at home. These can help increase your recovery speed and help prevent future injuries and misalignment.

We recommend completing the your chosen set of exercises 2-3 times through, morning and night, unless instructed otherwise by your physician. If you feel pain with any of your assigned exercises we suggest you discontinue doing that specific exercise.

If you have any specific questions about any of these exercises, please feel free ask your doctor or one of our chiropractic assistants.
